Position Summary: We have an exciting opportunity to join our team as a Senior Service Coordinator.
In this role, the successful candidate is a LMSW Social Worker with a background in working with older adults. The purpose of service coordination is to foster an environment in which older adults can live independently and remain in their communities safely and with dignity as long as possible. The Senior Service Coordinator oversees the service coordination program for three subsidized senior housing developments in Southwest Brooklyn. The Senior Service Coordinator will also carry a caseload and be responsible for various other activities within the onsite programs, including Friendly Visiting and caregiver support.
Provides routine individual and group supervision to Service Coordinators at three sites located in Sunset Park and Bay Ridge, Brooklyn.
Establishes and maintains quality practice procedures for the provision of service coordination, in compliance with U.S. Housing and Urban Development (HUD) and NYC Department for the Aging (DFTA) standards.
Provides ongoing, relevant training for service coordination staff as needed.
Oversees and coordinates the Friendly Visiting program.
Acts as liaison between service coordination staff and building management.
Oversees tenant transitions to and from health care settings. Liaises with medical personnel, home care agencies, and discharge planners to ensure safety and continuity of care.
Seeks out and maintains updated information on entitlements and benefits, and resources for seniors within the organization and in the community.
Supervises social work interns as needed.
Provides support to the onsite Social Adult Day Services program including caregiver support groups and performing assessments.
Manages a caseload of older adults with complex physical and psychiatric needs, providing supportive counseling, case assistance, home visits, information and referral, and crisis intervention as needed.
* NY State licensed masters-level social worker from an accredited university * Minimum of 3 years post-masters experience * Experience supervising staff and/or social work interns * Experience working with older adults and comprehensive knowledge of issues related to aging, including entitlements, benefits and community resources * Must be computer literate
Preferred Qualifications: * Experience with group work * Bilingual English/Spanish, English/Cantonese, or English/Russian
* Standard office environment * Ability to see or access information in print and/or transmitted electronically * Ability to hear oral instructions * Performing job duties may require prolonged periods of sitting
