The Acute Care Palliative Care Nurse Practitioner possesses expertise in the management of complex responses of individuals, families and communities to identify actual or potential health problems. The NP serves as nursing leader, consultant, researcher, educator and direct care provider. The Palliative Care NP Collaborates with home health nurse, hospital nurse, attending and palliative care physicians and others as needed to discuss identified patient needs and to order medication adjustments, treatments, and other care.The NP within in the Nursing Leadership role provides a visible and accessible clinical presence to the health care team. As Consultant, the NP works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration or referral. As Educator, the NP educates the patients and/or families about preventive care, medical issues, and the use of prescribed medical treatments and/or medications. The NP as Researcher utilizes evidence-based medicine in clinical practice and uses research process and findings to improve nursing practice. The NP as Practitioner, effectively identifies, evaluates, and addresses disease prevention and health promotion issues while administering quality patient care. The NP takes responsibility for enhancement of his/her own professional development.
Licensed Registered Nurse in the state or New Jersey.
Certified Nurse Practitioner in the state of New Jersey.
Masters prepared in Nursing from an accredited program
Must be certified from an approved facility or completion of requirements within 90 days of hire for:
Minimum of 5 years' experience in nursing in an acute care or post acute setting with two years of experience as an Advanced
Experience in Palliative Care Nursing Preferred
Demonstrated ability to establish positive, collaborative relationships with physicians, managers and hospital staff is essential.
Well developed ability to prioritize and simultaneously manage multiple projects.
