Works under the supervision of the Regional Manager and Director, Case Management.
The Chronic Kidney Disease RN case manager serves in an expanded nursing role to collaborate with patients/families and their health care team including nephrology, Primary Care Providers, and other specialists, to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services across the continuum. Assesses, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patients health status. Integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of transitions of care plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare for high risk population.
Develops core systems of care that monitor patients across the health continuum with a focus on effective and safe transitions from hospital to home, nursing home or rehab with a goal to optimize resources and reduce avoidable readmissions back to acute care. Promotes effective handoffs to appropriate primary care and specialty care teams. Works effectively with other members of the health care team to optimize interventions.
Manages performance feedback metrics to further refine the care model to maximize clinical, quality, and fiscal outcomes for the targeted population.

EDUCATION AND/OR EXPERIENCE:
RN license or eligible for licensure in the state of Pennsylvania required. Bachelor's degree in health care preferred.
Obtain Case Management Certification within 3 years.
Minimum of five years experience in clinical nursing required particularly with acute care experience. Previous experience with case management preferred. Knowledge of the basic concepts and principles of managed care required.
Clinical experience with CKD preferred.
Critical thinking skills and ability to analyze complex data sets required. General computer knowledge and capability to use computers required.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Work is typically performed in the hospital and office environment. Some travel required.
