The Senior Services Home Care Physical Therapist plans, organizes, coordinates and controls a caseload of home care patients in assisted living and memory care communities.We are looking for someone to join our growing team.
Senior Services is a division of Fairview Home Care and Hospice that treats patients living in the Ebenezer assisted living, memory care and independent living communities. We began our Senior Services Rehab Program 4 years ago and nowsupport 24 plus assisted living communities throughout the Twin Cities Metro area in Minnesota. Our Rehab team has grown to29therapists currently who support these buildings in teams. In these settings our staff get to know each other well and have strong team collaboration, which improves communication and collaboration and in turn brings higher satisfaction to our patients. Recent employee engagement surveys from Senior Services physical therapists scored at 100 % asking if their work gives them a sense of personal accomplishment and they also scored 29 points higher on teamwork than the rest of their counterparts. This position will work with a team of OTs, STs, RNs, PTAs, and Social Workers developing a collaborative care plan for the patient receiving home care services under Medicare part A.
This role will be a Physcial Therapist CaseManager in our West Metro Ebenezer buildings including Plymouth, Orono, Minnetonka. Home base will be at our Minneapolis office off 26th Ave.
This position is 80 hours per pay period on the day shift.

To be successful the Physical Therapist with Home Care & Hospice one must be able to:
Accountable for managing the clinical episode of care to achieve desired patient outcomes and measurable functional goals.
Communicates consistently and effectively with patient, caregiver, significant others, physicians and all team members to assure appropriate implementation and completion of the patient plan of care.
Manages patient expectations.
Advances the POC with the multidisciplinary team. Consults with the other team members as needed to make modifications in the plan. Decreases utilization of services as patient moves towards goal achievement.
Supervises paraprofessional staff in patient care.
Attends and participates in patient care rounds.
Communicates/coordinates with Clinical Resource, Utilization Review and/or MA Specialists.
Maintains complete, accurate, and timely clinical records/ documentation on all patients according to agency policy. Completes documentation on the day of service.
Meets all compliance measures (ex: MD orders, insurance authorization). Timely submission of all documentation.
Regular communication with manager regarding concerns and problem solving.
Provides safe, effective skilled and advanced intervention, within scope of practice according to patient need.
Provides counseling and teaching of patient, family and caregiver regarding care needs and health promotion/prevention.
Evaluates and documents response to care/services delivered.
Demonstrates ability to adjust approaches to care to reflect developmental level and cultural differences of populations served.
stays current (expert) in home care regulation and all applicable licensing
actively participates in educational programs, meetings, community activities and agency committees as appropriate
accepts responsibility for the understanding and implementation of the discipline applicable content of all departmental communications and procedures
identifies processes to improve and potential solutions.
Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.
Partners with patient care giver in care/decision making.

Licensed as a Physical Therapist and eligible to practice in Minnesota by start date
Current BLS (AHA or American Red Cross) certification for healthcare providers
1 year recent clinical experience as a Physical Therapist
Valid Minnesota drivers license (or license that matches your current permanent residence).
Current and adequate automobile insurance policy
Computer skills: able to effectively document in EMR
