The Registered Nurse (RN) Case Manager is a valued member of the interdisciplinary team, responsible for the overall coordination of care throughout a patient’s time with LightBridge Hospice. The RN Case Manager identifies patient/family needs and provides hands-on care in accordance with the attending physician’s orders and the interdisciplinary plan of care.
The RN Case Manager will
Be knowledgeable in caring for the terminally ill and help patients live their final days with dignity and comfort
Possess strong interpersonal skills and ability to work effectively with an interdisciplinary team, community partners and physicians
Have demonstrated ability to assess and respond to the needs of patients, families and caregivers in a variety of residential/care settings
Form professional relationships easily and maintain professional boundaries
Maintain compliance with hospice regulations, documentation requirements and deadlines
