Work Hours/Shift: PRN/Day You Will Be Responsible For: · Responsible for the application of nutritional guidelines and standards in accordance with current practices, · Patient care activities include, but not limited to: · Reviews medical record to assess nutritional needs, · Accurately completes nutritional assessments, calorie counts, obtaining diet histories and food preferences, · Provides individualized nutrition care to meet the needs of the individuals and families within their home and community. · Instructs/educates patients and/or significant others on modified diets · Report pertinent observations and findings to the team supervisor and RN case manager as well as communicating this information to the attending physician. · Maintain clinical record information in accordance with established standards and within established time frames. · Assists and instructs the patient/family in the planning of patients’ menus, · Assists the department with special projects when requested, · Provides individualized nutrition care when appropriate for patients at nutritional risk using appropriate calculations and judgment, · Assists in departmental and community in-services/education when assigned/needed, · Upholds professional standards of performance and conduct including performance within guidelines and ethics of the Academy of Nutrition and Dietetics Practice Standards. · Excellent customer service skills. Qualifications
What You Will Need: - B.S. or advanced degree from an accredited institution with a major in Food and Nutrition OR Dietetics
- Successful completion of dietetic internship
- In the process of being registered within the first six (6) months from hire and must obtain registration within 12 months of hire by the Commission on Dietetic Registration
· In the process of being licensed within the first six (6) months from hire and must obtain license within 12 months of hire by the State of Florida · CPR Certification · Valid Florida Driver’s License · Automobile liability insurance Job Summary: The Clinical Dietitian works closely with the interdisciplinary Home Health Care Team to provide medical nutrition therapy that is integrated and compatible with the patient’s medical program goals, objectives and expectations in accordance with the Hospital’s Mission. Instruction is provided for patients in the patient’s home setting.
