Incumbent serves as the social worker for GEC Community Care, with primary emphasis on the Contract Community Nursing Home (CNH) program. The incumbent plans, organizes, and directs administrative, operational, and patient activities for the program. S/he manages the daily operation of the programs, develops policies and procedures for program operation, and prepares reports and statistics for facility, VISN, and national use. . Major duties include but are not limited to: Incumbent serves as the social worker for GEC Community Care, with primary emphasis on the Contract Community Nursing Home (CNH) program. The Contract Nursing Home Program Coordinator administers information and analytical systems to evaluate and enhance the quality of service provided to Veterans in CNH's. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, sponsor data, and patient satisfaction. Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services. The incumbent administers information and analytical systems to evaluate and enhance the quality of service provided to Veterans. Develops and implements information systems to track Veteran satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. As a Contract Nursing Home Program Support Coordinator, the incumbent stays abreast of current federal and local Directives regarding the CNH Program. The incumbent will maintain a knowledge base of current theory and treatment modalities, by reading professional journals, attending conferences, clinics, seminars, and teleconferences, keeping abreast of materials distributed throughout the VA, and remaining current on private sector sources. The incumbent serves as a liaison between the VA and Community Nursing Homes under VA contracts. S/he develops and fosters relationships with community partners to assist with system navigation, maintaining collaboration, and coordinating services. The incumbent will function in support of caregivers, Veterans, and family members throughout the medical center and in community settings. The incumbent performs a wide range of services and provides technical assistance. Technical assistance provided is based on monitoring activities and program reviews, and the development and analysis of information. In addition, the incumbent uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. The incumbent performs program analysis, development, review, research, design, and contact work with nursing homes. The incumbent performs work related to the review, monitoring, analysis, planning, and assessment of the CNH programs. Responsible for key project activities related to the planning, development, and implementation of these programs. A substantial portion of the workload directed by this position is carried out at one or more locations that are physically removed from the main unit under conditions that make day-to-day supervision difficult to administer. The incumbent manages the daily operation of the program, develops policies and procedures for program operation, and prepares reports and statistics for service line, facility, VISN, and national use. The incumbent collects analyze and reports program data to local management, Office of Community Care, and Office of Contracting.
