The incumbent is a Social Worker assigned to the North Florida/South Georgia Veterans Health System - Mental Health Clinic. The Social Worker's responsibilities include the assessment of mental health and psychosocial problems, often at a time when the Veteran is experiencing crisis and include the provision of short-term, solution-focused interventions and counseling with Veterans and/or their family members/significant others/caregivers. The Social Worker is knowledgeable of a range of treatment modalities and has the skills required to provide the appropriate level of care/treatment through psychosocial treatment and evidence based clinical/psychotherapeutic interventions. The Social Worker must be knowledgeable of mental health conditions and how they acutely and chronically impact Veterans day to day lives. The Social Worker must understand how to properly diagnosis mental health conditions using the most current version of the DSM and this understanding must be accurately demonstrated through clinical documentation in the electronic health record. It is the responsibility of the Social Worker, working within the interdisciplinary treatment team, to share in the development of a treatment plan with the Veteran based on this assessment. Duties and responsibilities include, but are not limited to: The Social Worker is responsible for the development of a patient centered treatment plan and developing achievable treatment goals with the Veteran, and when appropriate, the family members/caregivers and collaborating with all additional interdisciplinary team providers. The treatment plan will be documented in approved VA treatment planning software and in the electronic health record. The Social Worker is responsible for planning and organizing work, coordinating with clinical interdisciplinary team members, other professional and clinical staff, and management personnel. The Social Worker is responsible for providing, reviewing and coordinating all phases of the continuing care for Veterans served. The Social Worker recognizes and understands the intimidation of bureaucracy and will act as an advocate when needed to serve the best interest of the Veteran and family members/caregivers. The Social Worker will educate and encourage the veterans to advocate on his/her own behalf, thus fostering a sense of independence and empowerment. The Social Worker provides education related to navigating the VA system, use of community resources, entitlements, Advance Directives/Living Will/Durable Power of Attorney for Health Care, and Goals of Care Conversations. The Social Worker provides psychoeducation to Veterans, family members/caregivers and refers them to appropriate interdisciplinary team members or services for identified health education needs. The Social Worker provides information about VA benefits and refers to the appropriate service organization for assistance. Responsible for supporting the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility. Willing to serve on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor. Mindful of keeping their supervisor apprised of problems and recommended solutions to problems encountered in the incumbent's area of responsibility. Responsible for furthering one's own professional growth through continuing education and for ensuring continuing education appropriate to the area of assignment. Able to provide Assistance with the orientation and learning of less experienced clinical Social Workers and, if interested, to serve as a preceptor for social work students. Able to demonstrate knowledge and skill in the use of software applications for drafting documents and data management as well as other computer systems in use by the VHA. Able to maintain a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work standards, Joint Commission standards, and facility standards.
