The incumbent practices as an RN in the Home Based Primary Care Program (HBPC) at the Alaska VA providing nursing care to a culturally diverse Veteran population with complex, chronic, and disabling conditions. Duties include but are not limited to: Manages a primary caseload working independently to provide direct patient care in the home environment Responsible for comprehensive case management providing initial and ongoing assessment of unique nursing needs Demonstrates competence in phlebotomy, catheter changes, and wound/skin assessments Strong collaboration with HBPC interdisciplinary team members results in the development and implementation of individualized treatment plans Monitors, coordinates, and reports on outcomes and progress of the care plan Work Schedule: Monday - Friday, 7:30 am to 4:00 pm Compressed/Flexible: Monday - Friday, 6:30 am to 4:00 pm Telework: Not available Pay: Competitive salary, regular salary increases. Paid Time Off: Annual leave at the rate of eight (8) hours per biweekly pay period; Sick leave at the rate of four (4) hours per biweekly pay period; 11 paid Federal holidays per year. Retirement: Traditional federal pension (5 years' vesting) and federal 401K with up to 5% in contributions by VA.
