Primary Responsibilities:
- Responsible for planning, directing, and coordinating medical and health care services.
- Design and manage health care plans for members according to their individual needs.
- Acts as a liaison between members, their families, and primary care providers.
- Must be an effective communicator with members, families, health care providers, and the health plans.
- Educating members and families on disease management
- Provide health education, coaching and treatment decision support for members
- Meets all of the medical, psychosocial and financial needs of the member
- Accurately uses database for daily documentation of clinical information
- Receive information from consumers about specific symptoms and questions
- Conduct interviews to understand cause and effect, gather or review health history for clinical symptoms, and determine health literacy
- Ask open ended questions, identify significant from insignificant responses, ask about other diagnoses, probe for additional information, and redirect as necessary
- Identify appropriate interventions and resources to meet gaps (e.g., psychosocial, transportation, long-term care) based on specific consumer needs-Review and identify appropriate evidence-based information for consumers
- Revise plans of care or close cases based on consumer success toward goals
- Document goal attainment (e.g., proof points, completion of intervention to obtain goal, caller understanding of action plan) and enter data in appropriate systems
- Other responsibilities as assigned

Required Qualifications:
- Minimum High School diploma
- Current, unrestricted LVN or LPN licensure
- 3+ years of Clinical experience, resulting in excellent clinical skills and judgment
- Experience with inbound and outbound telephonic skills
Preferred Qualifications:
- Knowledgeable of working with HMOs and PPOs
- Bilingual English/Spanish
