Duke Homecare & Hospice Duke Homecare & Hospice offers hospice, home health and infusion services as well as serves as the home for the Duke Caregiver Support Program. Our Hospice team works closely with a patient's physician to provide comprehensive, individualized care in the comfort their home or at our inpatient hospice facility located in Durham. Our team of nurses, therapists, social workers, counselors, and chaplains help patients and their families manage advanced medical conditions. Duke Home Health provides services in a patient's home that include skilled nursing, physical therapy, occupational therapy, speech therapy, social service, home health aides, and pain and wound management. Our infusion team comes to a patient's home to administer intravenous medications, including antibiotics and chemotherapy, to children and adults. Our nurses, pharmacists, dietitians, and patient services coordinators work under the direction of a patient's doctor to monitor needs and treatment.

JOB SUMMARY
The Nurse Manager, Patient Care is responsible for directing, managing and overseeing clinical operations of multidisciplinary team and coordinate agency clinical programs and operational activities on a 24/7 basis for the Hospice program. Ensures delivery of quality patient care and compliance with clinical and administrative policy and procedures and regulatory standards.
MAJOR JOB RESPONSIBILITIES
Review and monitor staffing levels and productivity.
Evaluate clinical practice of specified discipline and review clinical records to ensure standards of care and quality of services are provided in accordance with Medicare, Medicaid and Joint Commission.
Maintain current knowledge on regulatory issues as they relate to Medicare, Medicaid and Joint Commission.
Ensure the agency is in compliance with all legal and regulatory standards.
Develop and implement agency Performance Improvement activities.
Monitor the Medicare Dollar per episode (Home Health), per diem (Hospice) amount patient.
Assist in preparation and monitoring the budget components for the clinical department.
Perform analysis of financial information, reporting trends and patterns; prepare educational activities and develop remedial action plans as necessary.
Facilitate the team process including the interdisciplinary communication and negotiation skills.
Assist teams in development of team goals and standards; evaluate the educational and competency needs of staff.
Coordinate identified educational and competency needs with clinical practice group of staff.
Manage and resolve concerns encountered by staff in the development of objectives and goals, annual budgets, physical facility, equipment and supply needs, organizational structure, work distribution and operational processes.
Serve in departmental on-call rotation which may include making on-call patient visits.
Conduct patient visits when necessary.
Conduct coaching conversations for team members.
PREFERRED QUALIFICATIONS
Education
Bachelor's degree in Nursing is required. Master's degree in Nursing strongly preferred.
Experience
Three years nursing experience required
Degrees, Licensures, Certifications
Must have current or compact RN licensure in the state of North Carolina. BCLS certification required.
Nurses hired into leadership positions must obtain a leadership certification within three months of hire. New nurse leaders who are ineligible to sit for an exam because they have not been working in a leadership role for the minimum amount of time required must complete their certification within three months of becoming eligible.
Valid NC Driver's License; Certification/Licensure as required by regulatory agencies.
Specialty certification per program preferred.
Professional Work Experience
Prior supervisory experience strongly preferred.
Working knowledge of Medicare and Medicaid regulations, including JC and DFS standards, ICD-9 Coding preferred.
Knowledge, Skills & Abilities
Computer skills including MS Office (Word, Excel and PPT preferred.)
Excellent communication and organizational skills required.
Work Location
Primarily on-site; some remote flexibility may be permitted
