We’re seeking a compassionate and proactive Registered Nurse to join our clinical team as a Care Navigator, coordinating wellness plans and supporting client success through consistent, personalized engagement.
Position Highlights
Manage a dedicated panel of clients, providing proactive wellness outreach and coordination.
Collaborate with physicians, APPs, and wellness professionals to deliver seamless care.
Document and track labs, screenings, and follow-up plans for continuity of care.
Support infusion therapy needs and backup IV coverage as needed.

Ideal Candidate
Licensed RN with outpatient, concierge, or wellness experience

IV therapy experience preferred but not required
